After completing this form, you will receive an email with a Fed Ex Label within 5-7 business days. 
Cobalt Medical Supply will charge a return shipping charge of $10.50 and a 15% restocking fee which may be deducted from any credit due, when applicable. All returns/damages must be reported to our office within 72 hours of receipt of goods and received in our warehouse within 30 days of receipt of goods or items will no longer be eligible for a credit. We do not accept returns for refrigerated items or expired products. All returns must be in original packaging.
